We participate with the following insurance plans:

If you have insurance with United Healthcare Community Plan, it is your responsibility to keep your contract with them current. If your insurance with Community Plan lapses or terminates, you will be financially responsible for any charges incurred for services provided by Westfield Pediatrics, P.A.
If your Insurance Company requires a co-pay, you will be responsible to pay it at the time of the visit. If you have any deductibles or co-insurance, you will be billed for the appropriate amount after we receive payment from the insurance company, YOU MUST BRING YOUR INSURANCE CARD WITH YOU TO EACH VISIT.
We require that you notify us of any change in your insurance. Please familiarize yourself with the benefits of your plan as well as the restrictions. You are ultimately. responsible for services that are not covered by your plan.
