As flu season approaches, we will be holding walk-in flu shot clinics in our annex throughout flu season as long as our supply lasts.  Our schedule of flu shot days is posted below.  We will be adding additional dates through the flu season as our supply permits.  We encourage all of our patients over 6 months of age to receive the flu shot. You may not receive the flu shot if you are allergic to eggs. 

We will not be scheduling appointments for flu shots during regular office hours.    

Please check this website frequently for updates on the scheduled dates of our flu clinics.  Also, as our flu vaccine shipments will be periodic throughout flu season, please check the website before coming to a scheduled flu shot date as if our supply is out, we will need to reschedule the session.   

The flu vaccine for 2011-2012 will contain the H1N1  vaccine as well.

To minimize waiting times, please print and complete this registration form for EACH child coming to the clinic and bring it with you.
